5. Service Quality & Consistency Pressure

The institution's service standards, customer experience, or quality consistency are eroding. Symptoms include rising complaints, declining satisfaction, and inconsistent delivery across sites or channels. The root cause is almost always a standards architecture that has not been engineered for scale or consistency.

Why Category Matters

The most common transformation failure is category confusion — treating a structural pressure with an operational intervention, or a systemic pressure with a strategic one. This is the institutional equivalent of prescribing antibiotics for a vitamin deficiency: expensive, ineffective, and ultimately corrosive to trust.
